12-14: 
  • Take vocational and recreational interest inventory
  • Complete travel training assessment
  • Obtain state identification card.  Dept of motor vehicles
  • Locate and confirm social security card
  • Explore Community agencies, recreational options
  • Job shadow for career exploration
  • Review accommodations needed for college
  • Visit College Board

14-16:
  • Obtain information about driving training and license
  • Discuss non-standard testing procedures
  • Obtain work permit from guidance
  • Investigate SSI, SSDI, and Medicaid (Regional SS office, AHEDD)
  • Obtain copy of birth certificate
  • Explore OVR (Office of Vocational Rehabilitation)
  • Apply for accommodations for SAT testing (College Board)
  • Register for PSAT (Practice SAT)

16-17:
  • Research adult living
  • Discuss estate planning, transfer of rights
  • Apply for SSI, SSDI, Medicaid 1 month before 18th birthday
  • If your child is receiving MA under the Disabled Child Category they must reapply as an adult through the local County Assistance Office
  • Explore community based work

18-21:
  • Inform health insurance company of child's disability
  • Obtain rider of continued eligibility
  • Reminder to check status with OVR
  • Visit post-secondary vocational vendors
  • Confirm transfer of rights and/or estate planning
  • Update resume and cover letter
  • Locate adult recreational/social centers
  • Confirm SSI and Medicaid
  • Confirm education, employment and individual living arrangements
  • Selective service registration for males
  • Encourage child to register to vote
  • Your child may be eligible for the Adult Autism Waiver thru Bureau of Autism Services
  • Your child may be eligible for the ACAP and other services provided by the Bureau of Autism Services
  • Your child may continue to be eligible for services via the Office of Vocational Rehabilitation
