In order for the State Board of Nursing to notify you when it is time to renew your license, it is necessary for the Board to have your correct name and address. If you changed either/both but have not reported these changes to the Board, please do so as soon as possible to assure that you receive renewal notification.
You can verify or change your address online at: http://www.mylicense.state.pa.us. Remember that licenses are mailed to the address on record and will not be forwarded.
To change your name, complete the Request for Change of Name application available at http://www.dos.pa.gov/nurse and mail to the Board with the required supporting documents.
Remember, even though practical nurses are not required to complete continuing education, in order to renew your license the Board MUST receive confirmation that you have completed two hours of APPROVED child abuse recognition and reporting training on the Child Protective Services Act. If you do not receive a renewal notice within 45 days of your license expiration date, please go to http://www.mylicense.state.pa.us or the Board’s website for renewal information.
